INJURY WAIVER AND GENERAL RELEASE FORM

By signing your name below as a parent/legal guardian of a participant in Youth Production rehearsals presented by Birmingham Children's Theatre at Altamont School, you acknowledge that participation in the program exposes your child to the possibility of personal injury. You hereby release Birmingham Children’s Theatre and its officers, directors, employees, consultants, independent contractors, affiliates, and Altamont School from any and all liability for property damage, personal injuries, and/or other claims arising from or in connection with your child's participation in the program, including claims that are known and unknown, foreseen and unforeseen, future or contingent.

You acknowledge that you have read and fully understand the Injury Waiver and General Release Form.

By checking this box, I hereby consent to the use of photographs/videotape taken at the Birmingham Children’s Theatre for publicity, promotional and/or educational purposes (including publications, presentations or broadcast via newspaper, internet or other media sources). I do this with full knowledge and consent and waive all claims for compensation for use or for damages.

*BCT reserves the right to move any camp to a virtual format at any time in consideration of ongoing COVID-19 developments and transmission rates in the county and state. Refunds will not be issued for classes that move to a virtual format.

Birmingham Children's Theatre Spring Class Registration Refund Policy

Thank you for choosing to enroll in Birmingham Children's Theatre Spring Classes. Please read our refund policy carefully to understand the terms and conditions for class registration refunds.

Deposits:

  • All deposits are non-refundable.

Full Refund:

  • The last day to receive a spring refund for Summer classes is two (2) weeks before the first day of your registered session. Requests for refunds must be communicated by this date.
    • Session 1: May 19th
    • Session 2: June 2nd
    • Session 3: June 23rd

Partial Refund (50%):

  • After the 2-week mark, you will be eligible for a 50% refund of the total class fee. Refund requests must be submitted in writing before the following dates:
    • Session 1:  May 26th
    • Session 2: June 9th
    • Session 3: June 30th

No Refund:

  • No refunds will be issued starting the day after the first day of classes.
